Visit Information

Before Your Visit 

• Establish your Weill Cornell Connect account 
  • For assistance with account setup, visit https://weillcornell.org/weill-cornell-connect  
• Complete the pre-visit questionnaire 
• Send us any relevant medical or genetic records through Connect or by email (GPCP@med.cornell.edu)  
  • Your pathology reports (if diagnosed with cancer outside of Weill Cornell) 
  • Your cancer genetic test results (if performed) 
  • Your family members’ cancer genetic test results  

During Your Visit 

You will meet with one of our genetics clinicians and review the following information in detail, as applicable:   

• Personal medical history, including cancer screening practices, previous genetic test results, imaging, and pathology reports.  
• Family history, including 1st 2nd and 3rd degree relatives, and any family member genetic test reports.  
• Genetic risk assessment and education including a discussion of your personal and family history, an evaluation of projected cancer risks, and an explanation of the potential outcomes of genetic testing. 
• Informed consent for genetic testing: this will cover information such as the type of results to expect, insurance coverage and billing for the test, and privacy laws.  Per New York State Law, a genetic test consent form must be signed prior to sample collection.  
• If you choose to proceed with testing, your provider will provide details on sample collection, result communication, and scheduling a follow up appointment.  
• If you do not proceed with testing, your provider will discuss your personalized cancer screening and prevention plan, as well as any relevant information for your family. 

Sample Collection 

For patients who proceed with genetic testing, the following applies: 

• Patients with in-person appointments can have their blood drawn for genetic testing on the same day. Patients with virtual (video) appointments will be scheduled for a blood draw, often on a separate day. 
• At the time of the blood draw, our dedicated program medical assistant will confirm the informed consent is signed. If not, they will collect your signature.    

Follow-Up Visit 

• Once available, the genetic test report is released to your Weill Cornell Connect Portal account.  
• If you have not already scheduled a follow-up visit with your GPCP provider, please contact our office to do so. It is important to complete this discussion with your genetics provider so that they can provide you with recommendations for cancer screening and/or prevention.
